Biomarkers of Low Ovarian Reserve May Not Predict Fertility as Previously Thought

Women with test results indicating low ovarian reserve were no less likely to conceive within six or 12 months of attempts than women whose lab tests didn't point to a limited egg supply, researchers report in JAMA.

Doctors use the tests to decide whether to recommend egg freezing as an option for women to preserve future fertility options, researchers note, and home-based tests are also marketed to women who want to assess fertility on their own.

Lab tests showing that women have low reserves of eggs in their ovaries may not necessarily mean they will struggle to get pregnant, a US study suggests.

Ovarian reserve (or the number of remaining oocytes in the female body) has always been evaluated by measuring levels of antimϋllerian hormone (AMH) and follicle stimulating hormone (FSH) found in blood and urine.

The results showed that low AMH or high FSH had no bearing on the chance of a woman falling pregnant within any given month, and did not lead to a lower chance of conceiving after six or 12 months.

A new study found the results of the popular tests - which are created to tell women how many eggs they have left and their quality - had no bearing on whether women went on to conceive.

The tests, which can cost a few hundred pounds each, are offered at fertility clinics across the United Kingdom as a way of predicting response to fertility treatment. However, after accounting for differences in age, women with a low AMH value (low ovarian reserve) were just as likely to get pregnant within a year of trying compared to women with a normal AMH value (normal ovarian reserve).

"Fertility does decline as both men and particularly women get older and so if you start trying for a baby and think there may be problems, or if you've been trying for a year without success, don't delay before seeking advice from a fertility specialist, who will then guide you to the appropriate tests that are right for your personal situation".

Professor Richard Anderson, from the Medical Research Council's Centre for Reproductive Health, University of Edinburgh, said: "Many more women are concerned about having their first child when older than was the case for previous generations, leading to pressure to seek "fertility tests".

"This paper, confirming smaller earlier studies, shows that we do not have such a thing".

Women are born with a finite supply of eggs, and that supply dwindles with age.

"However, we found that they do not predict her likelihood of conceiving naturally", Steiner said by email. Tests for ovarian reserve biomarkers have become widely used as a growing number of women elect to put off attempting pregnancy until they're in their 30s and 40s.

Dr Channa Jayasena, clinical senior lecturer at Imperial College London, said: "This is an excellent and rigorously designed study".

"Hormone levels change with time, so taking a snapshot today tells us very little about what women's fertility will be like tomorrow". Steiner, M.D., M.P.H., of the University of North Carolina, Chapel Hill, and colleagues conducted a study to determine the extent to which biomarkers of ovarian reserve (early-follicular-phase serum antimullerian hormone [AMH], serum follicle-stimulating hormone [FSH], serum inhibin B, and urinary FSH) were associated with reproductive potential.
