After her release from the hospital, Demi Lovato has checked into a rehabilitation clinic.

Demi was discharged from the hospital on Saturday and reportedly took a private plane to the facility, which is located outside of California, according to TMZ.

A source told TMZ.com: "Demi's team held a meeting last week to discuss separating her from toxic people in her life once she gets out of rehab".

Sources close to the pop star, who turns 26 on August 20, tell TMZ that "there will be a major focus on separating her from toxic people in her life once she gets out of rehab - including even a distant relative who is believed to have a negative impact on her", the site explains, adding that there's one friend in particular who's at the top of the hit list.

"I've always been transparent about my journey with addiction", the singer says. In the new message, she shared news of her recovery in no uncertain terms for the many fans who admire her. "It is something I must continue to overcome".

The statement continued, "I want to thank God for keeping me alive and well. Your positive thoughts and prayers have helped me navigate through this hard time".

The singer-actress has spoken about her struggles with an eating disorder, self-mutilation, drugs and alcohol.

I now need time to heal and focus on my sobriety and road to recovery. Lovato relapsed upon her release, and she later admitted, "I wasn't ready to get sober".

She also thanked her family, team and the staff at Cedars-Sinai hospital in Hollywood, who she said had been at her side since she was admitted on July 24.

Lovato signed the letter, "I will keep fighting [heart emoji]".
